Description

DUTIES AND RESPONSIBILITIES:

1)      Assist Social Services Director with the admission of residents to facility.

2)      Promotes facility census growth and marketing strategies.

3)      Assists in the determination of room assignments and ensuring that residents,  families and all departments are informed.

4)      Ensure that all records required to be filled out are done so completely,  legibly,  and in a timely fashion.

5)      Assure regulatory compliance by encouraging appropriate record keeping and charting in facility and by maintaining appropriate files to document compliance.

6)      Assist Social Services Director in the documentation of,  follow up and tracking of grievances.

7)      Be able to prepare for and participate in multi-disciplinary Resident Care Plan Conferences as directed by Social Services Director.

8)      Provides information to residents and families of resident’s regarding Residents Rights and privileges.

9)      Perform casework with residents and families per the direction of the Social Services Director.

10)  Assist with the coordination of information which will enable resident to be discharged at medically appropriate time.

11)  Provide information to other facilities as directed by Social Services Director.

12)  Be aware of community resources,  and be able to refer appropriate residents to those resources.

13)  Participate in continuing education programs available,  with consent of Administrator.

14)  Participate in facility’s quality assurance program.

15)  Promote the residents’  well-being and satisfaction by supporting resident centered care.

16)  Follow all company Social Service,  Personnel,  and facility policies and procedures.

17)  Follow established safety and infection control procedures.

18)  Participates in all required inservice training.

19)  Maintains safe working conditions and follows dress code.

20)  Reports to supervisor all complaints and grievances made by residents,  families and visiting individuals.

21)  Adheres to and promotes Code of Conduct.

 

22)  All other duties as assigned.

Responsibilities

******  Must be COVID vaccinated or willing to complete a religious or medical exemption form.  Job would be pending until exemption is approved or denied.  If exemption is denied no job offer will be made unless the applicant receives COVID vaccine.
